Requesting PAC Code to move to another provider, do I need to wait until my contract is up?

Do I need to wait until my contract is up, or can I do this when in my final month? (To avoid any contract cancellation charge)

 

Contract is up on 28th September, what would be the best time to request PAC Code?

I'd wait until around the 10th as you need to give the new network 48 to 72 hours to process the port in @EncBlue

To avoid early penalty charges you need to wait until you have completed your minimum term.
