All you need is:
1 - A 4g phone (with 4g enabled in settings)
2 - A 4g sim card
3 - A 4g tariff
4 - Be in a 4g area.

You also need to check that you have a 4g sim card (the sim number will start with "4ggp") and also ensure you have 4g or LTE option enabled in the phones network settings option.

We assume that your 4G catchment area is properly up and running.
You can check with your postcode here http://status.o2.co.uk
As a process of elimination try your SIM card if it is a 4G type, in an alternative phone to see if you then get 4G symbol in the phones status bar.
