Need a Proof of Purchase

Hi,

I recently upgraded my phone and got a Samsung. As part of this, I can claim some money back from Samsung, but need to provide a Proof of Purchase which contains all of the following 3 items: Date of Purchase, Retailed Name, and Model.

Out of the 4 or 5 documents sent to me, I still can't seem to find a document which contains all of the 3 items.

Please could I get some assistance with this?

Log on to your MyO2 and get to the Download Bills section - in there, a section called "My Documents" should have all you need, @JV91 - Guide: How do I get to the 'Download Bill' page?
