You may be using the wrong form or completing it incorrectly.
Double check all details to make sure they are correct.

Your best option at this stage is to call customer service or the poorer option of using live chat

might be me BUT I didn’t get whether 2nd box was temporary sim number or the number you wanted to keep, so went via online chat and that was when got a text / email confirmation
they should have worded it bit better though maybe some checking for eroneous form filling
