So I ordered O2 sim card on 19 October 2016, and it was a 12 month agreement. I don't want to use it or pay for it anymore but I am still getting charged, how I am supposed to cancel my plan??

You can't cancel it now. O2 give you 14 days to change your mind. As you are now past that you have no option to cancel unless you pay the 12 months term.
