Its the only one I could find.
Do you have a more current version ?
O2 CS will confirm if the address etc is still valid and or send one out.

no always set it up on the phone never filled one in myself when we changed bank accounts just rang o2 and all was sorted

I used to pay my bills online, but O2 has decided make online payments impossible, I therefore need to fill out a new direct debit mandate and get it paid automatically.

HI,
Direct Debit is the best way to go and takes around 7 days for o2 to set it up.
But if you contact them on 202 they should be able to do this over the phone as Adamtemp64 did.

I disagree, it's much better to pay online, the direct debit always comes off at the wrong time of the month and O2 refuses to change the date to a more suitable date.
