I have just upgraded my phone and when i log on to my o2 i see my monthly usage bill but it doesn't show my device plan. For my old phone it was split in two.???

No, not at all, You will pay CPW directly for that and be covered by their own insurance T&C's not O2's.

If you upgraded directly with O2 it will still be split, but make take a few days to update your MyO2.
If you upgraded elsewhere, your device and airtime payments are combined into one payment only.

I went to carphone warehouse to upgrade but am still on o2 will that make it one payment also with my insurance all showing as one payment.??

Insurance is still extra shown as a bolt on I think if O2 insurance, but totally separate if CPW insurance and yes, it's a standard tariff so combined device and airtime single payment.
