You need to contact O2.

When you call, you will be asked for the reason you are calling.
Stay silent for 15 seconds and you will be taken to the keypad options.
Press 2 for Everything Else
Press 5 for More Options
Press 7 for Anything Else which will put you though to someone.
You can also try saying "Upgrade" or "Fraud" to be connected to an advisor.
8.00am is the best time to call if you can.
Or pop in your local o2 store with ID.

Thats great, thanks for this. Its actually for an old Samsung that i had on O2 but my wife needs to use it with a 3 sim. i need it unlocked, when i put in the PUK for that handset it says PUK is incorrect

Unlocking is different Guide: Unlocking an O2 phone to use a different SIM card
but if the phone is asking for a network sim pin it may be hardlocked.

Thank for the reply - ues, I filled the form in and they sent an unlatching code whihc we entered carefully and correctly 3 times and it never worked. I phoned to tell them this and was sent the exact same code. Its driving us mad. I have read that I may need a MUK code - but no idea how to get this?

Yes, it's hardlocked now.
It's an MCK you need so I'll tag @Martin-O2 and @TheresaV who should be able to help you next week.
