I selected the 12-months contract last year in O2 online shop and it's coming to the end a few months later. However, I have left the UK earlier and never need any refresh of my own account. To avoid any further unexpected charge, I wonder if the contract would be automatically shut down after my last payment or still be refreshed.

The contract will continue to run until cancelled, but after the initial 12 months, you can cancel at anytime without additional fees.
You should call or write to O2 to cancel as you are out of the country.

If you don't want to keep your number you should write a letter and send this by RECORDED DELIVERY to the following address:
