My bill is always about £12. This month is almost three times that and I have not used the phone any more than normal. Why is this?

You may have called numbers not included in your tariff or gone over your included minutes, sent MMS messages etc,
You can keep a track of your useage in your MyO2 as well as viewing bill details.
