Have an ipad which has come with a SIM. In a year it has never been used at all, I am still having to pay an additional charge of £13.00 per month for the unused Sim on top of the £12.75 for the device. Does anybody know if it allowed to be cancelled? It seems ridiculous to pay £13 for 48 months when it will never get used!

There may not have been enough time for the system to catch up or possibly your airtime hasn't been cancelled correctly.
You'll need to call customer service.

Or advice from an O2 Guru regarding a way to get through:
If you call 202, when it comes to the reason you are calling stay silent for 15 seconds and you will be taken to the keypad options.
Press 2 for Everything Else
Press 5 for More Options
Press 7 for Anything Else which will put you though to someone.
Say "Upgrade", "Lost/Stolen" or "Fraud" if you have to, to get put through to someone.

You can only cancel the sim once you have paid off the iPad.
