Have been experiencing intermittent network and data coverage for several weeks. In last few days an error appeared reading: SIM not provisioned and all network and data coverage has ceased. I have tried all troubleshooting fixes, e.g. reconnecting manually to data network, flight mode toggle, restarting, physically reinserted micro SD, re-mapped network by access point name etc. Nothing worked. Have contacted Virgin as SIM was previously with them and has now been ported to O2.

Sounds like you need to a new sim card.
You have 2 options.
Option 1 Call Customer Services from another phone on 0344 809 0202 and request a replacement sim card
Option 2 and the best one, is to go to an o2 store with photo ID, and they can issue you a new sim there and then.
