When wanting to use this option on the phone, and so choosing 'ON', the phone then asks for a PIN number before it can action the request.  If the user doesn't know the PIN number which is being asked for, or if they've forgotten it, how can this choice be opened?  Do forgotten PIN numbers stop the user from using those facilities on their phone, or could they be overridden somehow by someone who has the know-how?

The sim pin 2 is required to set up or disable the FDN list.

I believe the default sim 2 is 5555

I also believe so, but obviously should be changed to prevent unauthorised use.
