I cancelled the contract after 2 payments and paid off the handset but I'm getting direct debit collection attempts to take £319.09 as early termination.
Why is this?

I'd ring them from an O2 powered phone as I assume yours may have been turned off now if you've not moved to Payg etc.
Or from From a landline 0844 809 0202 > 5p

It should be as this on o2's faqs page.
"What if I want to leave O2 and end my O2 Refresh plan?
You just need to pay off whatever's left on your Phone Plan. Then you can either sign up to a new plan, or leave O2. If you'd like to leave O2, after you pay off your Phone Plan, ask us for your PAC code or give us notice to cancel your account."

Be sure to lodge a complaint (ideally in writing or by email). If you've been charged for bouncing a direct debit make sure to inform O2 of how much it was as they should cover this. I got my charge refunded and a (small) goodwill gesture when O2 tried to take money from my bank that they shouldn't have done. I'm also trying to get my bank to cancel any charges as it wasn't my fault
