Direct fallback to 2G from LTE did happen in some countries/regions, because that network did not have 3G, or that LTE is not designed to do CSFB on that domestic 3G, or 3G coverage is poorer than LTE coverage.

I wasn't actually aware there was any obligation for VoLTE ?
I have no affiliation whatsoever with O2 or any subsidiary companies. Comments posted are entirely of my own opinion. This is not Customer Service so we are unable to help with account specific issues.

The OP intimates that VoLTE is part of the Ofcom requirement which I wasn't aware of?

I think whether there is any obligation is not actually important. Last I knew, 4G is intended to be all-IP, using IP for voice as well is for efficiency so it will eventually comes out and also for emergency calls.
