Please follow the advice here Guide: Lost or Stolen O2 Device? - How to Report it
Regarding being named on his account, you can only do this if he is with you and gives permission. So your best bet is to ring customer services with him by your side, and speak to them Guide: How to find help & contact O2
Once you get the new sim, customer services will need to unbar his account.
