I'm having a similar issue to a number of people. I got the Galaxy S6 which came with the £50 Google Play offer. After a few weeks I chased it up via the online chat and was advised that I'd have a code emailed to me with 'it takes 2-24 hours to receive but for you I'll do on urgent basis'
I left it three or four days and tried the live chat again, I was advised by someone else that the last advisor was unable to create the code due to system error and so it'd be forwarded to the concerned team. I had the following details from the chat
o2: I've forwarded the details to our concerned team and they'll get back to you with the positive result within 5 working days.

@ajscc from personal experience any credits won't show up on your account until your bill is generated. have you had a bill since the live chat in your post?
not sure about the rewards part, but @Toby has managed to resolve a few members issues, so hopefully he can work his magic again...
