The easiest and quickest way is to check in My O2.
Alternatively look on the website, call customer services or use Web chat.
options are limited at this time so My O2 will give you the code.

I went on the website to obtain PUK code just in case I need it. Instant answer - great. But I did not have to log in or pass any security questions. Unless the website knows my computer's ID via Cookies? Surely that is a huge security loophole? Is there much point to the PUK when it is so easy for a thief to find?

It is to me a bit of a security problem if there is no pin lock on the phone. However, most people who are savvy enough to have sim lock will also have phone lock too.
