Hello I have lost my phone and for insurance purposes I was to contact 02 to receive first date of usage and last date of usage on my previous number. I have tried doing this online and also called however can't seem to get this. Is this something you could help with.

You need to report the loss first Guide: Lost or Stolen O2 Device? - How to Report it

Then ask O2 for a standard insurance letter.

If you message O2 on Facebook (https://o2uk.co/O2CFB) , Twitter (https://o2uk.co/O2CTW) or Instagram (https://o2uk.co/O2CIG) , they should be able to help you with this
