The opening question I had in one interview was and bear in mind, I could see my name and the time they arranged on the paper in front of the person doing the interview: "What can I do you for?" Followed by them saying "oh I didn't realise it was an interview, have to re-arrange this, I'm busy".

Ah that's not good!
The thing I don't really like about interviews (in my experience) is that you are given a time to be there. You will always be early (I was an hour early once as I was unsure if the later train would get me there in time). I got there and I waited and waited and the interviewer didn't come until 30 minutes after the interview time. I have never had an interview start on time. Surely interviewers should show a good example to the people that they are interviewing?

Well to throw a real spanner in the works here, just imagine nowadays how people would react if they were asked some of the questions I was asked at interview....
In the 60's, as a nurse being interviewed for promotion, I was asked by the Consultant on the panel:
* Is your husband happy about the shifts you may have to work?
* Have you got any intentions of getting pregnant?
Even then I was furious about that line of questioning.. but I am afraid it was custom and practice 'back in the day'
