I have hit this issue in the past.
What I had to do was get the carrier to contact Apple who can validate the handset swap and thus change of IMEI.
You shouldn't have to pay for the unlock, as it should be free as you say.
Tricky part is getting hold of someone who is sensible enough to understand what has happened and then unlock it for you.
That rules out online chat - don't bother.
Best thing I can suggest is firstly try O2 customer support on the phone, explain to them what has happened, it is very simple and they have surely come across it before.
If you don't get anywhere with this, try one of the O2 staff members on here - see if they can help (Toby for example).

Hi,
Get a Payg O2 sim card from an O2 shop > top it up with £20 and make a few chargeable calls to latch your account with the iPhones imei number, and re fill out the unlocking form.
£15 for unlocking fee.
£5 for chargeable calls.
Or ask someone else with that sized O2 sim card on payg or contract to put their sim in it, make a few chargeable calls, and they fill out the unlock form.

The O2 chat advisor suggested going into an apple shop and asking them, but I don't think apple will unlock it for me will they?
You could get an adapter from an O2 shop and use that to make your nano sim into a micro sim, insert it into the iPhone 4s.
Make the calls again to latch it and fill out the unlocking form.
I assume you have been speaking to Live Chat !
Apple would not entertain unlocking any network locked iPhone im afraid.
