Scroll to the bottom of the page and enter the mobile number : http://www.o2.co.uk/puk/landing/
If it doesn't show there get her to sign into My O2 on the pc and the puk is recorded there.

A replacement sim is easy to get and just requires a call to CS to activate it once received.
Just contact CS for assistance if required http://www.o2.co.uk/contactus
