Solved: getting a PUK number to unblock a phone

I have been through the menu but keep getting caught in a repeating cycle of "CAPTCH" pictures and not giving me a PUK number to use to unlock phone

I then get told it is sending me a code vis text message but I cannot read as phone is locked!

No one on the end of live chat line..

Any ideas how I get get my phone unlocked please? cannot phone O2 as phone is locked!
