How do I request a new SIM card for iPad

If its a new iPad then you will to signup either to a PAYG plan for the iPad or sign up to a montly plan, all details are in the store. 

If you have lost the sim card that was provided then you need to call customer services on 0344 809 0202. 

This is not customer services and we dont have access to your account
