i rang o2 to tell them about my phone needed fixing i told them all that happend and they said they'd ring me back they never did. so i decided to send in the claim form to o2 insurance ive not heard anything nor have i had a replacement phone?

Call customer service again and they should fill out the form and arrange collection whilst you are on the call.

As @MI5 said, about How to submit a claim: Just call O2 customer services to submit a claim. You might be asked to fill out a claim form. Unlike some other policies we send you a replacement phone, so you can be back on track quickly.
