My Ipad has developed a fault where it continually black screens, I sent to apple but they restored it and sent it back as they could not replicate the fault. The day it returned within 4 hrs it black screened and became unresponsive. I have restored it 4 times since then but it is now dead with no response. Has o2 got any responsibility to replace it.

As long as it's still under warranty you go approach either O2 or Apple for a repair.

Apple are more likely to replace it though over O2 repairing it.
