bars and usage form

i been trying for over a week to get my bars and usage form from O2, 

And they always say that will take 24 hours for the email, and I call then several times and have not received yet.

I've not ever seen such a form but you will need to keep chasing customer services I guess ?

@AnonymousAre you saying your phone has been barred?

If so you will have to do as advised by @MI5 and contact customer services to get any bar lifted...

(if this is the problem)

Hi @Anonymous! 

Do you mean the "proof of usage" or "proof of purchase" forms by any chance?

 

These can be requested for example if you have insurance through a 3rd party and need to prove where you got your phone from. slight_smile
