Hello there
I redeemed my O2 extra today for Amazon Prime but I have accidently used login detail for a amazon account I don't use anymore from years ago, instead of the details from my current account
They use the same email address but have different passwords
Is there anyway to cancel the extra so I can start again and log in with the right Amazon account?

You can't add to an existing Amazon account so the only way you can use it is with a different email, as you have done already.

The O2 extra has been added to an existing Amazon account, just one I don't use anymore. I didn't create a new Amazon account to use the extra

Not one with Prime already it can't.
Nothing we can do here anyway, you need to contact O2 which probably won't result in the outcome you want either.
