where do you find the o2 iphone unlock form?

i clicked on a link from the help page but doesnt work, takes me to another help page

i just upgraded to iphone 6 (contract) and want to unlock my iphone 5 so i can sell it

any help appreciated

Looks like there's a problem with the link. You might want to send a PM to Toby and report it. Probably wouldn't get sorted at this time of night, so you'll have to wait and see if it's repaired in the morning.
