Unlock iphone form?

where do you find the o2 iphone unlock form?

i clicked on a link from the help page but doesnt work, takes me to another help page

i just upgraded to iphone 6 (contract) and want to unlock my iphone 5 so i can sell it

any help appreciated

Just to add that you will need to remove the 5 from your Apple id or you will have problems with each others texts etc.

Also make sure you sign out of iMessage and facetime, turn off find my phone and fully reset it before setting up with the new Apple ID.
