how to top up my contract phone number

Hi my name is Irene Yang.And my phone number is [Removed]. I realized it's overdue and I can't log in my account neither website nor app cuz I always logged in with the 4-digital code. But it's out of service atm. So can't top up myself. And mine is the contract number Could you please let me know how could I top up? and my account name is [Removed] I tried a few times seems like my account has been locked cuz i rly can't remember my password. So just wondering if it is possible to reset the PW or help me to top up my phone number.
