PAYG overcharging on auto top up

I signed up for the "Pay And Go Go Go Unlimited Texts + 150 Mins + 500M" tariff in April 2013, for the monthly charge of £10. The payment was until today taken by auto top-up. It is very unlikely the 150mins + 500M has ever been exceeded as the phone is lightly used and connects to wi-fi most of the time for data transfer. In the intervening 10 months I would have expected to have paid a total of £100, however these are the amounts that have been taken via auto top-up: 16/04/2013 15.00 25/05/2013 15.00 26/06/2013 15.00 29/07/2013 15.00 03/09/2013 15.00 14/10/2013 15.00 15/11/2013 10.00 16/11/2013 10.00 20/12/2013 10.00 21/12/2013 10.00 22/01/2014 10.00 22/01/2014 10.00 Total 150.00 Has anyone else experienced this issue? If you are listening O2 can you explain to me why I have been overcharged £50 in a relatively short period of time?
