I currently have an iPhone 6 Plus and am updating to a new iPhone 12 Pro Max.  Does my current Sim card (pay monthly) fit the new phone or do I need to obtain a new Sim?  Thank you.

Yes your sim will work.

If you want to use 5G, then you will need to request a new sim card and do a simswap when it arrives
