Depending where you were, @12RachelM, I can think of one case that would result in this - if you left your phone forwarding to O2 voicemail, as the incoming call, which in some places is chargeable if answered (by you or by voicemail), and then a call from your phone to voicemail for the caller to leave their message.
If you were in a country not eligible for EU "Roam like Home" or "Travel Inclusive", then had you had O2 Travel bolt-on (text "TRAVEL to 23336" message would have been sent to you), you'd have paid a maximum of £6 per day you were there for any day that you made a call or that your phone answered a call and sent it to voicemail.
Is that a possibility, @12RachelM?
