Follow the instructions on the link. Your name will be changed within 4 working days of receipt of the document. Hopefully that will be within 30 days of the date the account was opened. This can be checked using My O2, either app or on the Web page version.

Hi, unfortunately the change of name form is no longer available on the website. Pop in to your local O2 Store and they’ll be able to print one out for you, help you fill it in and if your bring your marriage certificate they might agree to fax/email both through for you.
Yes we mentioned a few weeks back that the form kept reverting to the 'contact us' page. Admin were looking into it for us. https://community.o2.co.uk/t5/Other-products-and-services/Name-change-form/m-p/1094375
Meanwhile this was given as the alternative
You can send a copy of the marriage certificate with a covering letter giving details of all the account numbers you have with O2 to
Correspondence Management Team
PO Box 202,Houghton Regis

You can also send copies via fax with the scanned proofs to 0870 6002402.

Yes...a good alternative as not everyone lives within reasonable distance of an O2 store. It's also one of many reported broken links on the website (which shows no sign of being fixed any time soon)
