Previous orders in myo2 work for the claim form.
Get a screenshot of your Samsung device order and upload it.
Delivery notes included with deliveries are accepted too.

@Christophe79

Did you buy your phone from O2 or a third party seller?

If a third party then you will need to raise a cashback claim from the 3rd party, eg Quidco / Topcahback etc.

Have you checked your spam folder?

I see it was this offer?

https://www.o2.co.uk/shop/samsung/galaxy-tab-s4#contractType=paymonthly

Buy selected Samsung Galaxy devices and you can claim up to £150 cashback. Fill in the redemption form within 30 days of buying your device. You’ll need to provide some details, plus a copy of your proof of purchase. Then you’ll get your cashback by bank transfer, within 30 days of your claim being validated.

So have you followed the instructions in bold above? The item should show in your orders in My O2. So that is all you need
