I have just upgrade my contract to a sim only deal (£10 10GB) which confirms free 6 months Disney+ pass but I have not received any confirmation, also as I already have Disney+ subscription, is it possible to add it onto my existing subscription?

Unfortunately you cannot add it or stack it to an existing subscription.

You will either have to cancel your current one or choose a different extra in your MyO2 http://www.o2.co.uk/myo2
