You need to speak to customer services http://www.o2.co.uk/contactus

Although this has to be done within 28days of purchasing an O2 mobile http://www.o2.co.uk/help/digital-services/o2-insurance

I believe you need to go in store so they can inspect your phone but look for alternative insurance too as there are cheaper policies available from other companies.
I have no affiliation whatsoever with O2 or any subsidiary companies. Comments posted are entirely of my own opinion. This is not Customer Service so we are unable to help with account specific issues.
