@AnonymousContact customer service. http://www.o2.co.uk/contactus Stay on the line until you get to the menu to report a lost or stolen phone. Also report the loss to the police. If you know the IMEI number of the phone it's always a help.

If your insurance is with O2 you will also need to call customer service to make a claim so a police reference number is definitely needed.
