What phone are you trying to unlock? If you have an iPhone 6 or 6+ on a Refresh contract you would have to pay off the device plan in full. If you have bought the phone on a third party contract you would have to pay off the entire contract amount. If it's neither of those phones, follow the link @Cleoriff has provided in the previous post.

If it isn't an iphone 6, is it P&G or contract? Call customer service if you don't want to use the form but it's normally the best way to get the phone unlocked as long as you meet the criteria.
