If your contract phone is insured by O2 Insurance, you will see the charge for this in your monthly bill, look in your MyO2 Guide: How do I get to the 'Download Bill' page?
It may also be that you have cover for mobile phone loss, theft or damage on your household contents insurance, or if you pay for your banking services (check your bank account t&cs for this).
If you are asking this because your phone is lost or stolen, O2 will provide a letter for your insurer once you report the loss to O2 Guide: Lost or Stolen O2 Device? - How to Report it
