MCK needed?

Hi,

I'm trying to use a Smarty SIM with my mum's Samsung phone so she can move away from her O2 PAYG sim.

I'm getting a "SIM network PIN blocked. Enter SIM network PUK." screen come up when I put the Smarty SIM in the device and have seen a few posts on forums saying this means I actually need a MCK number to unlock the device? 

I've got a PUK code from Smarty and a PUK and unlocking/unlatching code from O2 but none of these have worked and I don't want to enter an incorrect code again and risk freezing the device entirely so it's useless as it's my mum's only way of getting in touch with people. 

Please can anyone advise on what my next steps should be here?
