Why is the puk number within MyO2 device details showing a code for an old iPhone?
How do I get it to update the puk number for my new phone so I can unlock it?

The PUK is valid for the sim card, not the phone, so it does not matter what phone it shows as long as the sim is the same.
Or, do you mean you need a PAC?

Or finally, do you want to unlock your phone from the O2 network?
First check if it is locked with another network sim and if so, follow the guide here Guide: Unlocking an O2 phone to use a different SIM card
You can enter your phones IMEI manually if the default settings are incorrect.
