You have to call O2 and it has to be done within 30 days of purchase.

@sahana You don't have to have insurance from O2. Many banks do mobile phone insurance, or if you have a homeowner's insurance policy your phone may already be covered. There are many ways to insure your phone. You can shop around on Google. O2 is only one way, and not necessarily the cheapest or the best.
