How do I add my teacher discount to my pre order for iPhone 12?

You can't. The discount won't be available immediately on the new iPhones. It will be open in 3 months I expect. You will be able to add it then.

It will not be possible to add any other discount if you buy at launch as you need to add the others within 28 days of purchase.
