If yu don't have your phone to get it from it may be under device details in your MyO2 and will be on the box the phone came in if you still have it.
Failing that you can ask O2 for it. If you message O2 on Facebook (https://o2uk.co/O2CFB) , Twitter (https://o2uk.co/O2CTW) or Instagram (https://o2uk.co/O2CIG) , they should be able to help you with this
