Questions about “Keep your mobile number” form

I wish to transfer from O2 PAYG to monthly plan (SIM only) and wish to keep my original mobile number,

I will order a monthly plan SIM.

I was told to fill in the “Keep your mobile number” form for keeping my old mobile number after receiving the new monthly plan SIM.

Some questions about filling this  form:

1. “What kind of tariff are you on”, in my case should I select the original tariff (PAYG)? or the new plan (Pay Monthly)?
2. “Your mobile number” is it my original mobile number of PAYG, or the new Monthly plan mobile (which I will not use after transferred to my original mobile)?
3. Will there be any payment I need to pay for this “keep my mobile” process?
