What is the code to lock the SIM card,? preventing others from using my phone

I think I tried that and 1other, now I have one chance left. What happens if I lock the SIM?

Then you would need your PUK code.http://www.o2.co.uk/apps/getPUK/welcome

If you lock after that then you would need a new sim card...

Did you ever change the pin from default?
If you did and can't remember it you will need to get a new sim card.
Any O2 shop will swap it for you and the sim would then have the default pin on it.
