2 MIG number submissions made and 3 weeks later still no fix. Various calls to O2 support who promise to fix the issue and contact me but neither has happened

Try using social media if you have it @stephenmcginlay - you need to ask for the CLI (caller line identification) bolt on to be added to your account.
Once done, it should look like this:
