used against mobile no given on the form.
What does that mean/what do I have to do to get it unlocked?

When you unlock an o2 Iphone you have to provide the phone number that it was originally provisioned with, that is the reason you got that response in the email. o2 will have a record of the IMEI number and the mobile number that it was first activated on.

It doesn't need to be the original mobile numBER, just the last number associated with the IMEI on the o2 network. This will only show on o2's system if the number was used in the UK.
This is quite an old post however so I don't think it will now help the OP.
